If you have certain plans offered by your employer, we have exciting news....NOW YOU CAN LOSE WEIGHT FOR FREE at Universal!

HealthAmerica is now offering the Learn & Earn Wellness Education Refund Program. ALL members and nonmembers who carry HealthAmerica heath insurance can take advantage of this program.

If you join the TAKE IT OFF or  the WEIGH WAY LESS PROGRAM at Universal's Nutrition & Weight Loss Center, HealthAmerica will refund 100% of the program cost. (Up to $350 per calendar year, which more than covers the cost of either program!)

Follow the steps below:
 

1. Check to see if your HealthAmerica insurance plan covers the "Learn & Earn Refund" Weight Loss program.

2. Select and join Take it Off or Weigh Way Less at Universal Athletic Club (see a membership representative or stop in the Nutrition & Weight Loss Center to determine which is the best program for you).

3. Pay for the program up front, and keep your receipt or proof of payment. After completing the weight loss program, fill out this Learn & Earn Refund Request form. You can pick this up in the Nutrition and Weight Loss Center.

4. Have your weight loss coach sign the Learn & Earn Refund Request, include a copy of your receipt and mail to the address specified on the form.
